Hasn't really been ran since release, here's my suggestion for it. This champion has eviscerated 4 and inhibited, when eviscerated ends this equipment is destroyed. When this triggers or if the champion is destroyed while equipped, a poison serpent is summoned to the closest available space under control of the equipment's owner.
