When Coragh died I had 6 zombies out, counting his spawns, still I didnt get a swarm when I killed a unit with one of them.
Yeah it's been happening for a while. Seems like the game for some reason thinks you don't have 5 zombies despite displaying the icon for swarm, deploying another zombie to trigger the 'role call' seems to fix it.
